Why is Case Management important to your overall care?
HIV is not a “death sentence.” People living with HIV are now living long, fulfilling lives compared to the first decade of the disease in the 1980s. It is important to access medical services early in HIV disease in order to prevent premature onset of AIDS. HIV case management can assist you in finding the right medical team, as well as identifying other resources and services to improve your overall well being. The Project’s case managers will complete a comprehensive assessment to help you prioritize your needs and work with you to create and monitor a care plan to reach your health goals.
What is the process for becoming a client?

How do I know if I qualify for The Project’s Care services?
The Project serves all people living with HIV/AIDS. For some of our services, such as the food pantry, information/referral and mental health, this is the only requirement. The first step to seeing if you qualify for services is to meet with one of our Program Specialists to complete an assessment of your needs. Many of the Care programs have income guidelines and serve individuals who are 200% of the poverty level or below. The following chart is an example of allowable incomes for program qualification: Because this is only a guideline, you may still be eligible for services even if you are over-incomed.
